How a score is made
The one idea everything else rests on. Nothing on this tab is a fact about a patient.
How the scoring works
There is no model and no training data. Every barrier starts at zero, each rule that matches adds its weight, and the total lands in a band. That is the whole calculation.
What a weight means
One to four, assigned by judgement rather than fitted to data.
- +1ContributesReal but rarely decisive on its own.
- +2MeaningfulThe most common weight. Two of these reach Moderate.
- +3StrongOne of these plus almost anything reaches High.
- +4Can be decisiveUsed once in the whole model, for having no caregiver at all.
What the total means
A barrier scoring zero does not appear at all. Everything else bands:
- Watch1 to 2Worth knowing about. Plan for it, do not chase it.
- Moderate3 to 4Active work. Somebody should own this.
- High5 or moreLikely to hold the patient up unless addressed.
